Endometriosis is a common problem which affects young women in the reproductive age group. In this, the tissue which lines the womb (endometrium) is found growing in abnormal locations such as the ovaries or around the uterus. Every time the patient has her menses, there is also bleeding inside this. As a result thick dark blood accumulates in the ovaries (endometrium cyst) and there are blood spots all over the pelvis which cause the intestines tubes and ovaries to stick to each other. Endometriosis can cause severe pain before or during periods, also heavy or abnormal bleeding. As it damages tubes and ovaries, it often causes infertility (inability to conceive). The condition always worsens with time and in severe cases can result in total damage to all pelvic organs causing a frozen pelvis. The diagnosis is made by history, examination and sonography. Sometimes a laparoscopy (Key hole surgery) may be required to diagnose and treat endometriosis at the same time. The treatment is essentially surgical. Nowadays, 2-3 visual cuts on the stomach wall are used to introduce a camera and operating instruments inside the pelvis and the endometriotic areas are removed and burnt using current or laser. Despite surgical removal, there is a possibility of recurrence of endometriosis, and the same patient needs multiple surgeries within a few years. Medical treatment is used to create artificial menopause as when there are no periods, the endometriosis subsides. This can be done

Dr. Thomas P. Habif, MD discusses Acne Treatment Antiandrogens and Birth Control Pills. See more at www.dermnet.com PLEASE RATE AND COMMENT!!! Antiandrogen treatment of acne Antiandrogen treatment may be considered for adult women. Those who fail systemic antibiotics, relapse after a course of isotretinoin or for women who have symptoms of hyperandrogenism are likely to benefit. Symptoms of hyperandrogenism include irregular menses, infrequent or light menses, mild hirsutism, oily skin and androgenetic alopecia. Women with premenstrual flares, adult onset acne, acne worsening as an adult and acne on the mandibular line and neck may also benefit. Women with ovarian cysts and hirsutism may also benefit. Patient selection Failure of oral antibiotic Recurrences after isotretinoin Irregular menses Light menses Mild hirsutism Increased oiliness Androgenetic alopecia Treatment There are two approaches. Combination oral contraceptives ( COC) reduce circulating androgens. Spironolactone is an peripheral androgen receptor blockers. The two drugs are frequently combined to achieve optimal results. Oral contraceptives All COCs can be effective for mild to moderate acne. They are often ineffective for more severe cases. COC suppress pituitary gonadotropins. This results in ovarian hormone suppression. They also raise circulating sex hormone binding globulin which decreases circulating free testosterone. All of today’s low-dose COC s are estrogen dominant and are beneficial in women

Laparoscopic cyst removal by Dr. Jay S. Nemiro- Part II

Here you are watching the end of the right ovarian cyst removal and the instillation of Hyskon, a high molecular weight sugar, that has the consistency of motor oil. The process of putting in Hyskon is to prevent adhesion formation while the ovarian tissue is healing. Hyskon coats the tissue, not allowing surrounding tissue to “stick” to the ovary while it is healing and thus reducing the risk of scar tissue or adhesion formation. Since Hyskon is a sugar based substance, it is absorbed in 7-10 days, while it takes normal tissue about 5-7 days to heal. Hyskon does not compensate for “experience and expertise” but adds as an adjunct to good meticulous surgery.
